PURPOSE
You can use this:
- If you had your testamur destroyed, lost, stolen or damaged and require a new copy and you can use the following steps to apply for a replacement testamur.
Please Note:
- A statutory declaration outlining the circumstances of the loss or damage to the testamur must be included with the lodgement of this form. A damaged testamur MUST be returned to the Student Administration office before the request can be processed.

You will need to update all the fields marked with an asterisk (*) in order to complete the form.

- There is a charge of $120 for a replacement testamur.
- Additional charge of $30 for an international postage.
- After submitting this form, you will be redirected to our PayPal account where you can pay for your testamur using a credit card or your own PayPal account.
PROCESSING TIME:
Applications will be processed within 10 working days.
